Facilities and Storage Requests

Facilities work order services consist of building alterations, repairs, maintenance, cleaning, and duplication. For example: lock duplication, power outage, plumbing, heating, painting, moving, and E-Waste/trash disposal. The Business Services team will review the request and complete the Facilities form. If the work order is billable, the Business Services team will contact the requestor and manager. All work orders need to be tracked for status and for billing purposes.

Facilities Requests

To request a work order

  1. Email busserv@sfsu.edu with “Facilities Request” in the subject line.
  2. Provide the following information:
  • Name of requestor
  • Phone number
  • Detailed description of work requested. Example: If flooding, where it is coming from (faucet, sink, etc.)?
  • Location of work needed (building/floor/room number). Example: If a restroom, which stall?

Facilities building access request process for members of the University community (including contractors/special consultants).

To request building keys/card keys

  1. The requestor must submit a Service Ticket with the location and key number (or card key number for additional building access), and Incident # (required for lost key replacement).
  2. The requestor's manager must provide a justification for master keys and access expirations for contractors/special consultants.
  3. The Business Services staff will generate and process the key/access card request form for either the Main or Downtown Center (DTC) campuses.
  4. The Business Services staff will notify you by email or by phone when the key is ready for pickup.
  5. The requestor must bring an SFSU ID card or photo ID/Driver's License to pick up key(s) in Corp Yard.

Lost or Stolen Access Card/Keys

The key holder is liable for its safekeeping. Stolen/lost/misplaced card or keys must be reported immediately to Facilities & Service Enterprises at 415.338.1568 or the DTC. Additionally, an Incident Report must be filed with the University Police Department (UPD). For assistance, contact busserv@sfsu.edu.

Note: For DTC/College of Extended Learning (CEL), the cost for replacement is $100 for a cyber key, payable to SFSU, or $20 for a card key, payable to Westfield Center

Return Card/Keys:

  • Defective key: Facilities Corp Yard or DTC CEL & Westfield Center
  • Leaving the university: HR during Employment Clearance
  • Moving to another department: Facilities Corp Yard or DTC CEL & Westfield Center
  • No longer need access to ITS infrastructure/rooms: Facilities Corp Yard or DTC CEL & Westfield Center

Storage Keys Management

Storage Keys Request

For assistance, email busserv@sfsu.edu or create a ticket at service@sfsu.edu. Provide the following information:

  • Name
  • Email
  • Key number
  • Location (room number)
  • Phone number; personal mobile is optional

The Business Services team will process the request.

For Business Services Only

Storage Keys Checkout Procedure

  • Check IT key log file in the share drive
  • Check the key log number and key number
  • Issue key copy to the employee (original key must be kept for duplication)
  • Update record:
    • Spreadsheet - enter date, username, and location
    • Note folder - Write date, username, and location
    • Envelope - Write current number of keys

Duplicate Storage Keys and Record Keeping

  • To duplicate storage keys, request approval from Sarah
  • Assign a requisition number from the Procurement log file
  • Send request to Facilities website; provide billing information
  • Once copies are received, update Key Log file:
    • Spreadsheet - Enter date, username, and location
    • Note folder - Write date, username, and location
    • Envelope - Write current number of keys
    • Update Procurement log file - mark exact date the request was completed
